Because healthcare doesn’t stop at the hospital doors.
Caring for Our Communities’ trained Community Health Coaches provide free, one-on-one support to individuals and families, helping them overcome barriers to better health — from transportation and food access to vital local resources.
In partnership with healthcare providers, public health, and local organizations, they help connect people to the right support at the right time.
